Digital Accessibility Document Remediation Request

Digital Accessibility Document Remediation Request

Faculty can use this form to request an accessibility audit and remediation of course-related materials by the FDIC. This service is designed to support you in making your course materials accessible to all students.

How the Process Works

To help you build your own digital accessibility skills, we ask that you upload only one document of each file type (e.g., one Word document, one PowerPoint).

We fix the sample: Our team will fully remediate your uploaded file.

We provide a guide: We will return the updated file with detailed notes explaining exactly what we changed.

You apply the steps: You can use our notes as a template to update the rest of your similar course materials independently.

If you get stuck or would like to schedule a 1:1 consultation, please reach out!

Accepted Formats

This service applies to course materials created or adapted by faculty, including:

PDFs

Word Documents

PowerPoint Presentations

Excel Spreadsheets

Kaltura Videos

External Websites/Software

Before You Upload

Self-Evaluate: Please review the materials you plan to submit and learn more about compliance here: Digital Accessibility Compliance (right click, open in new tab)

Acknowledge Faculty Responsibility: While we are here to help improve the documents you submit, faculty are ultimately responsible for ensuring that all components of their courses meet accessibility standards. By submitting this form, you acknowledge and agree to these conditions.

Before You Submit: A Note on Alt Text
We strongly encourage faculty to write and apply their own alternative (alt) text for images. Because you know the specific purpose and context of your visual materials best, you are in the strongest position to write accurate, meaningful descriptions.

Please provide all requested information below so we can process your request efficiently.
